Prior to your appointment, we recommend completing your health history forms online. If not completed in advance, please arrive a few minutes early to do so in-office. During your visit, Dr. Diamond will request any necessary diagnostic imaging and conduct a comprehensive dental examination. A dental hygienist will also perform a professional cleaning to ensure your oral health is thoroughly assessed and cared for.

    When you visit one of our dental offices for the first time, our dental team will take the time to better understand your smile concerns, medical history, and lifestyle to better align with your dental goals and needs.

  2. Imaging

    Digital imaging is how our dentists understand your unique anatomy and needs. X-rays and digital scanners give us a full view of your teeth and oral structures to better serve your smile.

  3. Dental Exam

    Our dentist will take a look at your teeth, gums, and oral health, assess the current condition of your teeth and make any notes based on our visual exam to add to your file and inform future treatment.

  4. Treatment Plans & Follow Up

    After your exam, we will review your needs and outline a treatment plan that fits your lifestyle and priorities. Our team uses advanced equipment and years of experience to create a plan that supports long-term oral health.

Insurance

If you have dental insurance, please bring your insurance card and documentation so we can add it to your file.

Health History

We will review your medical and dental history, including medications and past treatments, to ensure your care is safe and personalized to your needs

New Patient Registration Forms

 If you have not completed your forms online, please arrive a few minutes early to fill out the new patient registration forms in the office.
